Sunrise Health Care Inc is a Colorado Program Approved Service Agency (PASA) providing person-centered supports that honor each individual’s dignity, independence, and rights. We work closely with individuals, families, guardians, and case managers to design services that fit each person’s goals, including community-based supports and reliable non-medical transportation that help people stay safe, connected, and engaged in their communities.

One-on-one support to help individuals build skills, access resources, and navigate daily life in their homes and communities.

Supported Community Connections

Small-group, community-based activities that promote social connection, skill-building, and meaningful participation in everyday community life.

Non-Medical Transportation

Safe, reliable transportation to day programs, appointments, work, and community activities, coordinated around each person’s schedule and support needs.

Residential and Host Home Support

Person-centered residential and host home settings that provide a safe, stable home environment while supporting daily living skills, health, and community inclusion.

Transportation Services

Wheelchair Accessible Transportation

Professional, wheelchair-accessible transportation using ramp-equipped vehicles, with trained staff providing safe boarding, securement, and support from door to door.

Ambulatory Transportation

Safe, reliable rides for individuals who walk independently or with assistance, including support with entering and exiting vehicles at covered pickup and drop-off locations.

Who We Serve

Sunrise Health Care supports individuals receiving services through Colorado’s Home and Community-Based Services (HCBS) waivers by providing person-centered support services and reliable non-medical transportation, including:

Developmental Disabilities (DD) Waiver

Adults with intellectual and developmental disabilities who require ongoing, comprehensive supports to live safely and participate fully in their communities.

Supported Living Services (SLS) Waiver

Adults who live more independently but need support services to maintain their independence and quality of life.

Children’s Extensive Support (CES) Waiver

Children with intellectual and developmental disabilities who require significant support services to remain safely at home and in their communities.

Non-Medical Transportation (DD, SLS, CES, CIH, CMHS, EBD)

Transportation services available to individuals receiving services through DD, SLS, CES, CIH, CMHS, and EBD waivers who require safe and dependable transportation to medical appointments, employment, community activities, and other essential destinations.
